This qualitative study explores community-level interventions to improve diabetes care in South Africa, focusing on public health facilitators and barriers. Conducted in Ngangelizwe Community Health Centre, Eastern Cape, the study used face-to-face interviews with 20 diabetes patients.

Key findings:

Facilitators for better diabetes care included community health workers, strengthened health education, and peer support groups.
Barriers included lack of support programs, food insecurity, and limited community-based health promotion initiatives.
Health promotion interventions should integrate community engagement, public health expertise, and local healthcare resources to improve diabetes self-management.
The study emphasizes the need for structured diabetes care programs that incorporate education, behavioral change, and local health resources.
