This pilot mixed-methods study assessed the acceptability and adherence to an ad libitum Mediterranean diet in women with Polycystic Ovary Syndrome (PCOS) over 12 weeks. Among 12 participants, adherence significantly improved, along with diet knowledge, cooking confidence, and time management. Thematic analysis using the COM-B model revealed facilitators (health benefits, simple guidelines) and barriers (external influences, food access). Findings suggest that individualized support and professional training are key for successful dietary interventions in PCOS.
