This study investigates patient pathways in primary health care systems in Norway and Canada, focusing on decentralization, coordination, and continuity. Through interviews with 19 health professionals, the study identifies three key themes: structural challenges, specialization of primary care, and the potential of dedicated personnel to improve continuity. Findings highlight differences in funding, management, and electronic systems as barriers to coordination. Recommendations emphasize the need for integrated strategies to reduce fragmentation in care delivery.
