This study explored how adherence to the Mediterranean diet (MedDiet) affects the physical component of health-related quality of life (HRQoL) in active adults aged 41–80. Higher MedDiet adherence, particularly in those aged 71–80, was linked to better physical health outcomes. Reduced adherence was mainly due to low consumption of vegetables, fruits, legumes, and fish. These results highlight the importance of maintaining MedDiet adherence in later life to promote optimal physical well-being.
