In 1,924 ICU patients ≥65 years with CAP (MIMIC-IV 3.1), mortality was 37.7% at 30 days and 59.1% at 1 year. Lower GNRI at admission was independently associated with higher short- and long-term mortality in multivariable Cox models, and restricted cubic splines showed a linear inverse association. GNRI categories (Q1<82 to Q4>98) stratified risk effectively. Implication: GNRI is a simple, prognostically useful nutrition screening tool for elderly CAP patients. [119+ source]
