This Iranian cross-sectional study of 203 overweight/obese adolescents assessed adherence to the Diabetes Risk Reduction Diet (DRRD) and its impact on metabolic health. Higher DRRD adherence was associated with significantly lower odds of being metabolically unhealthy (MUO), even after adjusting for confounding factors. Adolescents with high adherence had lower fasting blood glucose, triglycerides, and insulin resistance. The association held across genders and was more pronounced among adolescents with obesity. The findings highlight DRRD as a promising dietary intervention for improving metabolic health in youth.
